Information

Admissions

An Admission Representative is available 24 hours a day, 7 days a week.

The Admission Representative will process all necessary paper work for:
• Admission to the Emergency Room
• Admission to Out-Patient Surgery
• Admission to the Hospital

The following information will be needed during the admission process:
• All active insurance cards
• Social Security Card
• Picture ID
• Advance Directives (if you have these) – Living Will or Durable Power of Attorney
• Complete physician order for the service needed

During the admission process you may be asked to sign the following:
• Consent for Treatment
• Authorization to bill your insurance

Medicare recipients will also be asked to sign:
• Medicare Secondary forms
• Medicare Rights Statement

All co-pays and deductibles are due at the time of service.

We want the admissions process to be hassle free.
